Who attends:

  • Someone with Mild Cognitive Impairment (MCI)

  • Someone who needs supervision during the day

  • Someone with physical or mental limitations

  • Someone who may be socially isolated or lonely and feels more comfortable in a supportive setting

Requirements:

A member should be mobile with assistance of a cane/walker or wheelchair and usually continent

Hospice is a way of caring for the terminally ill that helps them to live their final days to the fullest, as free as possible from disabling pain and mental anguish - usually in the setting of their own home.

Hospice enhances the quality of a dying patient's life; it emphasizes the dignity and sense of peace of the individual and the strength of the family; it provides symptom control, interdisciplinary care, support and counseling; and it involves the patient and family to help them through illness and death, including mourning, grieving, and bereavement.

Criteria for entering the Hospice Program:

  • Patients for whom cure is no longer a realistic expectation and whose life expectancy is estimated by the doctor at 6 months or less

  • There must be a willing and able care-giving person (or persons) responsible for 24-hour-a-day care

  • Patient must have a primary physician who will accept the responsibility of the overall care

  • Patient must live in the geographic area we serve

  • Patient should, when possible, be able to particpate in decision making

To enter the Hospice Program, the patient, family members, physicians, or anyone associated with them may make a referral to Hospice. The Volunteer Services Coordinator will contact the family to arrange a visit.
